General Information
You do not need
a physician referral to see one of our doctors however it would be helpful to
us to have your physician send us any relevant medical history.
At the time of
your visit we will also ask you for a history of your present illness and any
medical/surgical conditions that you may have had previously. Although this may
seem redundant please bear with us. All information can only help us to serve
you better.
If it helps, please
feel free to bring a list of your conditions, medications, and any other information
you deem important.
We believe each
patient is an individual. Your treatment plan will be prepared just for you and
will be discussed with you fully in a private consultation. You will be fully
informed of any changes or revisions made to your treatment plan during the course
of therapy.
If you have friends
or relatives who have received similar therapy and their treatment plan was different
this is not a cause for concern. Your treatment plan is for you and you alone
thus it will differ from others.
Once you have begun
treatment, our staff will explain to you step by step, how the treatment is performed.
If you have any
special needs please let us know so we may accommodate you personally and make
your visits with us as comfortable as possible.
